Covid-19 Needs Assessment Sample

As the Covid-19 Disaster continues, more local people are in need and more people are mobilizing to help meet those needs. Toward that end, a survey will be sent via email and social media to gage directly from citizens what their needs are at this time. Please help us refine the survey before mss distribution. 

 

Please take the survey and reply by Friday, April 10 at 5 p.m. with answers  to the following questions:

 

1. How long did it take you to take the survey?  
2. What questions did you not understand?
3. What questions did not have an answer that fits your situation?
4. What suggestions or general response do you have regarding the survey?
