The App has autofill assistance for location and date, and the ability to upload photos of the households, which will be automatically mapped,so we will know which homes received the literature.
A contact log that follows. It is a repeating form inserted for the Community Health Workers to record basic info about follow-up calls from that household.It is a form within a form, and they canmake as many of them as they need to record follow-up contacts with the household.We could develop that form. As it is, they just type in notes.
The Priority Questions follow. We can edit or refine these questions, but we can't delete them.
The questions that follow the Priority Questions need to be edited. They were compiled from the surveys we envisioned for the FaithBuilder project and the EPA project to address hurricane recovery, and Indoor Air Quality before the pandemic hit.
We can add links to all of the outreach materials, so a Community Health Worker should be able to access everything through their phone App that was in the literature drop
